FSI membership for financial advisors is affordable! Annual membership is only $199. Also, it’s automatically pro-rated, so you will only pay for the remaining months leading up to a 12/31 renewal. You may join and pay online by CLICKING HERE. You may also pay over the phone at 888-373-1840.

While you should always confirm with your tax professional, we believe our $2,500 and $5,000 deductible plans are HSA qualified.

  • When you enroll you will be asked for your checking account number and routing number. All monthly premiums are paid via ACH deduction, generally between the 25th day and 28th day of the month before each next covered month. The monthly draft date is the same for all members.
  • If you need to update your ACH, call 800-425-9374 and provide the new information.
  • If, for some reason, your payment is rejected or comes back as ‘insufficient funds’, you will receive notification via email. Be sure to resolve non-payment by calling 800-425-9374, so you don’t risk termination of benefits. 

If you enroll at least two weeks prior to your effective date, our goal is to have your physical cards to you by your effective date. You will receive an email from claims@AMPS.com within 48-72 hours after your first premium payment is drawn. This email is important, as it contains your welcome information and a link to create your member account within the Vault Admin Services/AMPS member portal. Within your account you will be able to access your virtual ID card(s), eligibility record, claims, benefit documents and other important information regarding your healthcare.

Your card will provide PHCS network and Vault information. If your doctor or facility is not in PHCS, then our partner AMPS will take over to pay your claims. You can reach AMPS at 800-425-9374
